ORGANIZED HEALTH CARE ARRANGEMENT [Florida only]
Federal privacy laws allow healthcare providers and entities that collaborate to provide and manage a common group of individuals’ healthcare to become members of an organized health care arrangement (“OCHA”). GuideWell-Sanitas I, LLC, Westchester General Hospital, Inc. d/b/a Keralty Hospital, Florida Behavioral Center, Inc. and Guidewell Emergency Medicine Doctors, LLC participate in the Sanitas Organized Health Care Arrangement (collectively, the “OHCA Participants”). The purpose of the OHCA is solely for compliance with HIPAA and creates no legal representations, warranties, obligations or responsibilities beyond HIPAA compliance. The OHCA allows participants to engage in joint activities, including treatment, care coordination, healthcare operations as well as quality assessment and improvement, involving the use and sharing of protected health information related to the collective patient population and community. The OHCA provides the ability to share PHI throughout the OHCA as necessary for treatment, payment and healthcare operations. The OHCA Participants agree to abide by the terms of this Notice with respect to PHI created or received through participation in the OHCA.
Nothing contained in this Notice is intended to suggest that any OHCA Participants are agents of the other participants or that participants are liable for the acts or omissions of other participants.
CHANGES IN OUR PRIVACY POLICY
We reserve the right to modify this Privacy Policy at any time, and any modifications made will be effective immediately upon posting. We will post at the bottom of this Privacy Policy the date that modifications were last made, which should alert you to any changes since your last visit to the Site.
